How the procedure works

Riboflavin (vitamin B2) drops soak into the cornea and are then activated with UV light, forming new molecular bonds between collagen fibers that make the tissue stiffer and more resistant to further bulging. Two approaches exist: "epithelium-off," where the thin surface layer is removed first for deeper riboflavin penetration, and "epithelium-on" (trans-epithelial), which is gentler but may be less effective in some cases — your surgeon recommends based on corneal thickness.

A treatment to stop things getting worse, not to fix them

It's important to set the right expectation: CXL halts progression, it isn't designed to reverse the corneal thinning or improve your prescription. Some patients do see modest visual improvement as the cornea regularizes slightly, but the main goal is stopping the disease where it is now. Will I need CXL again in the future? Re-treatment is uncommon but possible if progression resumes years later, which is why periodic topography checks continue after treatment.
